Plain language summary: Use FormSlug for legitimate purposes, with your own Google Workspace account and a real custom domain. Don't create slugs pointing to harmful, deceptive, or illegal content. We can terminate accounts that violate these terms.
These Terms of Service ("Terms") govern your use of FormSlug, a Google Workspace Add-on that allows you to create branded redirect slugs for Google Forms URLs. The service is operated by Creytens.com BV (KBO/BCE nr. 0423.339.078, VAT nr. BE0423339078), Sint-Michielslaan 2, 8200 Brugge, Belgium ("FormSlug", "we", "us").
By installing or using FormSlug, you agree to these Terms. If you are using FormSlug on behalf of an organisation, you represent that you have authority to bind that organisation to these Terms.
FormSlug is exclusively available to users with an active Google Workspace account (formerly G Suite) associated with a custom domain. Personal @gmail.com accounts are not supported. You must be at least 18 years old to use the service.
To use paid features, you must also have a custom domain that you control and can configure DNS records for.
Your FormSlug workspace is tied to your Google Workspace domain. The person who first installs FormSlug becomes the workspace admin and is responsible for all activity within that workspace, including activity by members and managers they invite. The admin role can be transferred to another teammate at any time via the dashboard's Settings page; only one admin exists per workspace.
FormSlug uses a 3-tier role model. Capabilities are summarised below and documented in full at docs.formslug.com:
You are responsible for maintaining the security of your Google Workspace account. FormSlug is not liable for any loss resulting from unauthorised access to your account.
You may use FormSlug to create redirect slugs that point to legitimate Google Forms URLs that you own or have authorisation to share. You may not use FormSlug to:
We use Google Web Risk to automatically screen destination URLs. Slugs pointing to URLs flagged by Google Web Risk will be blocked immediately and without prior notice.
Slug names are allocated on a first-come, first-served basis within your domain. We reserve the right to reclaim any slug that violates these Terms, infringes third-party intellectual property, or is reserved for operational purposes (e.g. api, admin, login).
You do not acquire any ownership rights in a slug name. FormSlug does not offer any guarantee of perpetual availability of any specific slug.
FormSlug offers a Free plan and paid plans (Pro, Team, Business). Paid plans are billed per workspace domain as a flat monthly or annual fee, or per seat. Pricing and plan limits are published on our website and are subject to change with at least 30 days' notice to existing subscribers.
Paid plans include a 14-day free trial for new workspaces that have not previously subscribed. Only one trial per workspace domain is permitted. Cancelling a subscription permanently consumes the trial and cannot be re-used.
Payments are processed by Stripe. By providing payment details, you authorise us to charge your payment method on the billing cycle you select. EU VAT is applied automatically based on your location and VAT registration status.
Because paid plans include a 14-day free trial, no refunds are issued after a subscription begins. If you did not use a trial (e.g. a grandfathered or manually provisioned account), you may request a refund within 14 days of your first charge by contacting hello@formslug.com. Annual subscriptions cancelled mid-term are not refunded.
You may cancel your subscription at any time via the FormSlug dashboard or by contacting us. On cancellation, your workspace downgrades to the Free plan: your slugs are deleted, any custom domain is disconnected, and all team members other than the administrator are removed (Free is single-user). The administrator's Free workspace remains, and you can create new slugs up to the Free limit. To permanently delete your workspace, use Settings → Danger zone in the dashboard. After explicit deletion, all workspace data is retained for 30 days (in case you change your mind), then permanently removed. You may request immediate deletion within that window by contacting us.
FormSlug and all its components (software, design, documentation) are the intellectual property of Creytens.com BV. We grant you a limited, non-exclusive, non-transferable licence to use the service in accordance with these Terms.
You retain full ownership of your data (your slugs, your Google Forms, your analytics). By using FormSlug, you grant us a limited licence to process that data solely to provide the service.
FormSlug is provided "as is". We do not warrant that the service will be uninterrupted, error-free, or that any particular uptime level will be maintained.
To the maximum extent permitted by Belgian law, FormSlug's total liability for any claim arising out of or relating to the service is limited to the amounts you paid us in the 3 months preceding the claim. We are not liable for indirect, consequential, or incidental damages, including lost profits or lost data.
Nothing in these Terms limits liability for fraud, gross negligence, death, or personal injury.
We aim to maintain high availability but do not guarantee any specific uptime. We may modify, suspend, or discontinue features at any time. For material changes that adversely affect paid subscribers, we will provide at least 30 days' notice.
You may stop using FormSlug at any time. We may suspend or terminate your account immediately if you materially breach these Terms, particularly in cases of abuse, illegal activity, or non-payment.
On termination by us for cause, no refund is issued. On termination by us without cause, we will refund any prepaid amounts for the unused portion of the current billing period.
These Terms are governed by the laws of Belgium. Any dispute that cannot be resolved amicably shall be submitted to the exclusive jurisdiction of the competent courts of Bruges, Belgium.
If you are a consumer under Belgian law, you also have the right to use the EU Online Dispute Resolution platform: ec.europa.eu/consumers/odr. However, as FormSlug is a B2B product (Google Workspace required), most users will not qualify as consumers.
We may update these Terms from time to time. We will notify active users by email at least 14 days before material changes take effect. Continued use of the service after the effective date constitutes acceptance of the revised Terms.
